CAMPER APPLICATION

Attending Camp Daybreak is an easy process. Below we’ve broken out the application process into easy-to-follow steps:

Step 1. CLICK HERE to complete the Camp Daybreak camper application. Please use as much detail as possible. Incomplete applications will not be considered. Please mail complete applications to :

Step 2. Once your application is received and the applicant has been accepted for the upcoming session (please allow until June 15th for us to review your application) you will receive a packet of forms that include: a copy of a photo release, a physician form, a medication form and a list of things to bring to camp.

Step 3. Return medical forms and photo release form by due date.

NOTE: Campers will not be admitted to camp if they do not have the completed forms.
